Medical Maintenance - Consent tab

Use the Consent tab to maintain records of medical consent provided by parents.

 Opening the Consent tab

To open the Consent tab:

  1. Open the Medical tab from one of the following maintenance windows:
     Current Student Maintenance. See Current Student Maintenance - Medical tab in the Current students manual.
    • Future Student Maintenance. See Future Student Maintenance - Medical tab in the Future students manual.
    • Past Student Maintenance. See Past Student Maintenance - Medical tab in the Past students manual.
    • Staff Maintenance. See Staff Maintenance - Medical tab in the Human resources manual.
  2. Click .
    The Summary tab of the Medical Maintenance window is displayed.
  3. Click the Consent tab.
    The Consent tab of the Medical Maintenance window is displayed.


Medical Maintenance - Consent tab key fields and buttons

Fields

Field

Description

Consent Given

Select to indicate that consent has been given.

Consent

Type of medical consent given. For example, consent to administer paracetamol. See the luConsentType lookup table in the System maintenance manual.

Consent Given By

Family member or community member that provided medical consent.

Note:
Consent can only be provided by community members that are contacts or siblings.

Consent Given Date

Date medical consent was given.

Consent Expiry Date

Date the medical consent record expires.

Modified By

Community member that last modified the selected medical consent record.

Modified Date

Date the selected medical consent record was last modified.

Con. Seq

Unique identifier given to the consent record.

Consent Comment

Comment about the consent given.

Active Only

Select to only display active allergy records.

Buttons

Add a new medical consent record.

Delete the selected medical consent record.
