Duplicating forms

To duplicate a form:

1. Click .
   The My Forms window is displayed.
   
2. Click .
3. Click for the form to be duplicated.
   The following menu is displayed.
   
4. Select Duplicate.
   The following window is displayed.
   
5. Type a name for the duplicate form.
6. Click .
7. Repeat steps 2 through 6 to duplicate additional forms.
