- type of support service
- status of the support service
- support service details
- parent and staff contacts.

Lodged By | Community member that lodged the student support service application. |
Application Date | Date the student support service application was lodged. |
Details | Details about the student support service. |
Referral | Referral details for the student support service. |
Parent Contact | Parent contact for the student support service. |
Responsible Staff | Staff member responsible for the student support service. |
Service Details | Description of the student support services provided. |
Service End Date | Date that the student support service ends. |
