Description
The luImmunisationFormStatus lookup table is used to define statuses for immunisation forms.
The lookup table is used to populate the Form Status field on the Immunisation tab of the Medical Maintenance window. See Medical Maintenance - Immunisation tab in the Medical details manual.

Description | Description of the immunisation form status. |
CEOCode | Status of the immunisation form as registered with the Catholic Education Office. |
ExternalSystemType | Code for the external system that provides the lookup table data. |
ExternalSystemCode | Code for the immunisation form status used by the external system. |
| SynergyMeaning | Code to tell Synergetic how to treat the selected immunisation form status. See SynergyMeanings. This field currently only has one value: Complete. You can assign Complete to any immunisation form status to define the 'Immunisation Complete' status; selectable in the Immunisation tab - Form Status field to indicate student is fully immunised. See Medical Maintenance - Immunisation tab. |

ModifiedUser | User that last modified the lookup table entry. |
