Plan Status | Status of the individual plan. See the luIndividualPlanStatus lookup table in the System maintenance manual. |
Start Date | Date the individual plan starts. |
End Date | Date the individual plan ends. |
Review Date | Review date for the individual plan. |
Reason for Plan | Reason for the individual plan. |
Staff Member Responsible | Staff member responsible for administering the individual plan. |
Other Staff | Other staff relevant to the individual plan. |
